Hi, I have a question if any one can help it would greatly be appreciated. If the nurse practitioner see's patients in the hospital and they Dr puts an attestation that states,"I saw, examined, and evaluated the patient following the APP. I reviewed the note documented above and I agree with the findings, exam and plan of care. Is this sufficient enough to bill under the Doctor vs the NP.
 
Our MAC (Noridian) requires that the physician document a "substantive" portion of at least one of the three key components of the E/M service. I look for either an element of history, exam, or MDM documented separately by the physician in addition to their attestation note to support the shared service. I basically look for documentation to support that they actually shared the service and had face-to-face contact with the patient.
