Wiki NP/PA 'incident to' wording in progress note

Do you know of any examples of correct wording for NP/PAs to prove 'incident to' in their progress notes? Example: 'Under the direct supervision of Dr. Smith who sees the patient when appropriate and is in agreement with the treatment plan'.
 
Good afternoon!
In our practices we encourage (require :-) ) the providers to document their presence (as required). For example: I saw patient A with Nurse Practioner B and performed the examination, etc. of the patient. I have discussed and agree with the assessment and plan.

Of course if there are any variances to the assessment plan and treatment, then that will be documented.
