Wiki NPI/Referring Provider Box 17 on HICFA

I have a general question and I am not sure if someone is able to answer me or direct me somewhere. Box 17 & 17a require a referring provider if a consult is performed, etc. However when there is a Physician Assistant or Nurse Practicioner (not MD or DO) - are they (PA or NP) able to refer since they also have an NPI or does it have to be an MD/DO? Or could this be an insurance specific matter? Thank you for any help.
