Wiki NPI/Referring Provider Box 17 on HICFA

I have a general question and I am not sure if someone is able to answer me or direct me somewhere. Box 17 & 17a require a referring provider if a consult is performed, etc. However when there is a Physician Assistant or Nurse Practicioner (not MD or DO) - are they (PA or NP) able to refer since they also have an NPI or does it have to be an MD/DO? Or could this be an insurance specific matter? Thank you for any help.

Physicians (MD and DO) and the following non physician practitioners: 1) nurse practitioners (NP); 2) clinical nurse specialist (CNS); 3) physician assistants (PA); 4) and certified nurse midwives (CNM) are the only types of providers eligible to refer/order services or items for beneficiaries.
