Can anyone advise on the required documentation for Fetal Non Stress Test 59025 and is just signing the NST strip (by the provider) sufficient or does the provider need to actually lock the encounter themself within the EMR?
Can anyone advise on the required documentation for Fetal Non Stress Test 59025 and is just signing the NST strip (by the provider) sufficient or does the provider need to actually lock the encounter themself within the EMR?
The documentation should include the start and stop times, a hard copy of the strip, physician interpretation comments and of course a medical indication for performing it. As to how you document this in a way that would satisfy a payer should they ask to see it, that depends on your EHR requirements that attest that the provider did in fact do the interpretation. If there is a hard copy available to add to the EHR documents and the provider has signed it along with the interpretation that may be sufficient.
