Can someone give me some advice regarding these codes? Do you use the reason for the exam as the diagnosis? Or the findings? So far, all I've found is the LCD.
Appreciate any help.
 
For these I have always used the indication along with contributing factors. For instance, CAD as primary then secondary might be hyperlipidemia, diabetes, tobacco use, etc.
