Wiki Nuclear Stress Tests and PA's

We have a PA that supervises stress tests. We are getting reimbursment for 93016. We are not getting reimbursment for for codes: 93015 or 93018.
Can a MD read the stress portion and not be on site to bill appropriately or do they have to be on site?
Please advise
