Is it necessary to include more than the primary dx code for each CPT for payment purposes?

all dx codes applicable should be coded for the visit, the primary being the reason for the visit. It is necessary to develope a complete picture for the insurance company so we have to include all the applicable codes.
payment should not be the reason we limit our coding, we need to establish medical necessity and that usually is not easy to do with only one code.
