You may submit a maximum of 12. You may link only 4 to an individual CPT/HCPC II code.

Will Medicare allow Bilateral (RT<) items to be submitted on separate claim lines when there are different diag codes relevant to RT & LT?

I have listed it that way when there is no bilateral code choice. I have had no denials from Medicare.
