Wiki Nurse practioner did E/M and Dr did the injection same day - please help

Hello all -
Could someone please help me with question - The nurse practitioner saw the patient for her follow appointment and the patient wanted an injection which the Dr did performed now my question is how do I sent the bill? Do I sent the E/M services under the nurse practitioner and the injection under the Dr name?

Yes that's how I would bill it. If applicable, you may need to put modifier 25 on the e/m code. Even though they are different providers, they are in the same group practice.
