Need help with a code for a nursing facility patient. Our physician had a 1 hour multi-disciplinary conference with patient, family, nursing facility staff, OT/PT, and hospice to discuss patient treatment options, prognosis, and whether hospice was an option.

Any help is greatly appreciated.
