When a nursing home resident has an acute issue and is sent to the hospital, can pts primary care provider who sees them in the nursing home bill an initial visit?

Example: Patient is a nursing home resident who Dr. A routinely follows. On 6/5 pt went the ER and was admitted from 6/5-6/10. Dr. A sees them on 6/11 in the nursing home. Does this get coded as subsequent or go back to an intial visit? Are there any specific requirements that must be met in order to bill the initial again?
