If in the nurse's notes the nurse's tried to place a NGtube and the patient had an epitaxis, so they had a verbal order to hold the tube the nurse held pressure to the nose to stop the bleeding, do I need the Dr. to reference the exptaxis in the ed course? also they inserted a rectal tube I can not find a code for that, do I use an unlisted? thank you all any and all help is appreciated.
