OB diagnosis trimester doesn't match number of weeks

Is it required to query the provider when the trimester on the obstetric code does not match trimester calculation based on the number of weeks (Z3A...) code or can the coder change the "Z34..." or "O...." code based on the number of weeks. My supervisor is saying we need to query prover everytime.
