I am having a terrible time finding documantation guidlines for prenatal visits. The provider is only documenting minimal information on each visit. "Patient doing good". That is the entire note for the visit. I know the visits are included in the preganancy package but I am sure there needs to be some type of progress notes with some information documented! Please help anyone! Does the provider need to document anything in the prenatal visit notes?
