If a pt comes in the office that is pregnant and is only being seen for a UA that day should the visit be documented on the OB progress notes with the ACOG sheet, or should it be documented seperately on a progress note? OR...should all visits pertaining to the pregnancy be logged on the OB progress notes whether it be problems or OB care? Hope this was not too confusing:)
