I agree that Brett's article covers the basics, but information on this topic can also obtained directly from the CPT book, and ACOG/AIUM has published information on what should be included as well (but does not specifically address the codes). There is also a white paper done by ACOG/AIUM that addresses a detailed ultrasound. I have attached the files for these sources.
