Is the visit billable as a problem visit?

A patient sees her ob for routine antepartum visit.
Patient mentions she has had some palpitations. She had an ekg done which was normal.
Ob doctor documents that referral given to patient for cardiology specialist.

Yes, clearly this is a billable visit unrelated to the pregnancy. For leveling, you should only count the work/documentation that was not part of a routine antepartum visit.
