When a physician is seeing a patient for weight loss for excessive calories and they also have hypertension, diabetes, joint pain, sleep apnea what would be the primary diagnosis?

My understanding is we code the obesity code primary and then the other codes secondary due to they are comorbidities of being obese.

Can anyone help us with this issue? The physician and nursing staff are saying the comorbidities are the primary code not obesity.

Any help would be appreciated
