Is CPT Q0091 limited to certain insurance companies ? If not, when would we bill CPT 99459 since it is only $43.00/ Thank you

Our medical group will code the Q0091 across the board no matter what HP accepts and use it for Medicare for sure, but the reason the group decided to do this was to reprot for HEDIS .

" Notification: Effective June 1, 2025 except in the case of preventive E/M services, Cigna

will not separately reimburse CPT® code 99459 when reported with E/M CPT codes

99202 – 99205 and 99212 -99215 as it is considered included in the E/M service"
