I am coding observation codes for a practice. They are providing the date and time of admit and date and time of discharge on the Facesheet. They also provide progress notes for each time the physician visits the patient. However, they do not have "true" discharge summary. They provide a discharge plan but it doesn't even have the time of discharge on it. The time is on the Facesheet. How can I code, or is it even legal, to code the discharge day (99217, etc.) if they are doing it this way?
 
you should probably check the MCM rule on this, I thought it was required for inpatient and observation to have a discharge summary, but I could be mistaken.
