Hi,
Need assistance as for Observation coding, patient admitted 11.00 pm on 11/01 & EKG done @ 11.00 pm & for next day 11/02 another EKG done @ 1.00 am. We have to code both the EKGs as there is medical necessity, however, we should be not billing modifier 76/59 correct. Kindly assist.

Modifiers 76 or 59 would only apply to multiple procedures or tests done on the same date. Since the EKGs are on different dates of services, the modifiers are not relevant or necessary.

As a side note, I'd urge caution in making coding choices based on medical necessity. Only a physician or clinician is really qualified to determine whether or not something is medically necessary - it's outside the scope of coder training to decide this. Code what is documented unless your organization directs you otherwise.
