Can anyone tell me if these codes are supposed to be used when coding/billing in a hospital based outpatient Oncology office? We are new to Provider Base Billing and need some guidelines. Thanks.

We have not had any training on how to properly bill out our Medicare and Medicaid claims which will be going on a UB-04? We are not sure if the codes for infusion will get a facilty fee and technical. Do you have any guidelines for billing Provider base?

what revenue code are using for the physician and then is the staff under the facility payroll?

I have no clue on what revenue codes we are using for the doctors. The staff is paid by the facility, which we are now part of the hospital.

Im so sorry, instead of revenue codes, I meant to say condition codes.
