Hello-Can anyone share with me any insight regarding the total number of dx that can be linked to a cpt. According to my software vendor, the rule is only 4per CPT/line of service, with a total of 12 per claim. Is this correct? I was under the impression as many as necessary irregardless of cpt/line of service.
