If the destruction was exactly the same lesion as bx then you may code only the destruction. Also be certain the physician has documtented in the note that this is a neoplasm with uncertain behavior. This is not a code to be used lightly, and is derived via pathology to determine the behavior of the cells, whether benign, malignant or uncertain.
