Pt was seen in the office for E & M and 4 days later was admitted to hospital. Can I charge for both ?

Need more detail. Why was the patient admitted? Related to same condition?

I don't see why not, assuming you are the physician who saw the patient in your office and you also handled the hospital admission 4 days later.
