Question - One of our physicians received chart notes from a previous practice for one of their new patients. While looking through the EMR chart they realized it looks like this particular physician pretty much billed a level 4 for every visit. URI, pharyngitis, UTI etc. Now the physician is questioning if we bill to low. If you prescribe meds do you automatically bill a level 4?

Absolutely not. While the risk table is Moderate for the prescription you have to look at the entire documentation/record. Level 4 for this things is high. :rolleyes:
