Good afternoon. I am looking for some assistance and hopefully a black and white reference that I can show my docs in regards to my question!

We have a patient that presents to our office for an office visit on Monday. The doctor decides to admit the patient to the hospital on Tuesday for chemotherapy as an inpatient. For the date of the office visit, he is dictating the H&P to be used for the hospital admit and no office visit note.

I need to find documentation supporting the fact that he cannot use that "H&P" to bill both his office visit and the admit on the following day.

Since the patient had 2 encounters Office and Inpatient a day apart. Bill OP E&M visit with documentation, then bill another E&M for Inpatient admission. He is required to do 2 E&M, 2 DOS
