Can you bill the above scenario? Is it the same situation if a person goes to their PCP to have a lesion removed and you only bill the lesion removal? Or if there's another diagnosis and documentation to support a separate office visit, can you bill both with a mod 25 on the o.v.?

Yes, same situation. If the device ck was planned, then you can't bill for separate E&M unless there is a significant, separately identifiable svc. Make sure your documentation is solid when billing E&M with 25 in addition to other svcs.
