If a provider goes out into the parking lot and sees a patient in their car... is there a special way to code that?

As long as the parking lot is part of the premises of the office and the visit is being conducted out of the office with the support of the physician's own staff, equipment and supplies, then I think it would appropriate to code the office place of service.
I can't imagine there is yet official guidance on that situation, but I agree with Thomas. If the parking lot is your address, then that's close enough.
