If the docotr documents moderate-severe malnutrition how would you coudl it or would you query the physcian? I can't find the guidelines for this or I'm overlooking it. Thanks.

Query the doctor to verify moderate or severe as there are different codes for each.

Moderate - severe malnutrition is referring to malnutrition that has progressed from moderate to severe OR malnutrition that is at least moderate but has not yet reached severe. The provider would have to be queried for clarification.
