Oncology Clinical Note Examples

Our physicians follow the vague guidelines provided in the CPT book, clinical vignettes, however, I am looking for actual E/M note samples. I would like to present E/M note examples for each level of service. I have not been able to find any credible Radiation or Medical Oncology examples. Does anyone have the book Medical Record Auditor?
I was wondering if that book provided Oncology note examples.
