I would appreciate some feed back on this issue. For a patient that has been previously diagnosed with a malignant neoplasm, at what point does the diagnosis change from malignant neoplasm of _____________ to personal history of a malignant neoplasm of __________ (V code). Would this be a time issue ( # of years) or if the patient is still undergoing chemo or radiation therapy?

Thank you in advance for your help.
 
ICD-9 defines it as "a patient's past medical condition that no longer exists and is not receiving any treatment, but that has the potential for recurrence, and therefore may require continued monitoring."

Of course it states there are exceptions to this....but its on page 26 of the 2011 ICD-9
