When listing an onset date (for insurance companies), if the statement says "onset 2 days ago and still present" I would list the onset date a 2 days before being seen. If the statement says "onset date 2 days ago but worse today" I would list the day seen as the onset since the condition has worsened. Or, if the patient has had symptoms for 2 weeks, worse for 5 days, but seen by PMD today and sent over from office, I would again use the onset date as date seen as they were directed to the ED by their PMD. Are there any written rules on this that anyone is aware of? Or can I have a consensus?

Thanks
