when a surgeon does a hammer toe repair on the RT foot and than does it on the LT foot, do they need to dictate seprate OP notes or can the just same the same excate procedure was on the LT?

They do not need a separate note.

If one side was identical to the other, he can state that....as long as it is clear what was done.
