Are there any kind of Coding guidelines for Outpatient Therapy Coding; what can you code from for the Diagnosis. These are recurring accounts and the therapist is completeing the Therapy Evaluation Form. We also get a physician order for the therapy that has a dx on the order; but then there is an Evaluation form that will have a different diagnosis but this form is not signed right away by the provider. Can we use the evaluation form with the signature?
