Can anyone tell me how they would code the dx of "open wound of Lt toe"

They said it was not traumatic, therefore, this makes me think the L97.52_ codes. It is okay to assume the ulcer in non-traumatic wounds documented like this? Otherwise insurance companies want accident information.

If documented as non trauma then the provider needs to be equally clear as to if this is an abcess, or cellulitis or non pressure ulcer or pressure ulcer. No the coder may not assume this to be a non pressure ulcer.

This provider constantly puts wound everywhere in the documentation, and is very hard to confront..
