Hi All,
Is this an acceptable Operative Report for Debribement? in order for us to bill for;
11056=700 dx.
11721-110.1 dx.

Operative Report:
Exam Debribement of nails and callus.

Pls. help if possible an example of a brief operative report that I could use as a guide to the Podiatry Provider...

NO, all office procedures require a descriptive detailed procedure/surgical note with; anesthesia, prep, anatomical location, size, surgical instruments used, approach, method, clouser, wound dressing, post-op instructions,patient outcome/recovery.
