I've got an ophthalmologist who is billing for services using an E/M code rather than the vision codes (92002-92014).

Which are the correct codes to bill?

The reason the other codes were created because its harder to get a higher level for an eye doctor than most other physician specialties. So in addition to 95 vs 97 guidelines, you have 3 options and you pick the method that is the most favorable.
