How does one decide to assign an ophthalmology code 920XX vs E/M for a visit?
 
Check the Ophthalmology section in your CPT. It has very specific definitions of what constitutes ophthalmologic services. Your physician should be able to assist you.
 
Why is the patient coming for a visit. If it is a routine eye exam, most vision insurances will only pay for the 920xx. However, documentation is not as stringent for the eye codes and some docs use them for that reason, check with your doc.
