Dear Fellow Coding Professionals,

I have a client that wants to code shared services. They are a hospital based outpatient clinic and incident to does not apply.

Do you think the following statement is acceptable? The NP dictates the note and the physician follows with this statement:

"I have reviewed the history, labs, and necessary diagnostic imaging studies. I have examined the patient along with Jane Doe, NP"

The billing provider they want to assign as the physician. I thought personal documentation was required, and I did not see this acceptable.

It would really depend on the carrier requirements and if they accept incident to for an NP.

Novitas requires the provider to performed one of the three E&M components to qualify for split/shared visits. This documentation would not be sufficient to meet their requirements as we don't know what he/she examined.
