Is anyone else having issues with Optum denying and asking for refunds all of a sudden on Serum in Arizona? Just recently they have started denying 95165 for M51-Missing/Incomplete/Invalid Procedure Code. When I call they say it's because a modifier is missing or incomplete. We are not billing with any modifiers and there are no modifiers required when billing this code alone
