Wiki Oral Surgery-Excision vs Biopsy- What to code?

I am new working in Oral Surgery. If the surgeon excises a lesion for removal but then decides to send for biopsy I typically code the excision and include the biopsy. My question/Questions are: can you also bill the biopsy? If the intent is to excise but then the decision is made to send a specimen for pathology does the service billed then become a biopsy? Same with a debridement, if sent to pathology should the billed service then be biopsy? I typically go by the intent of treatment. Please let me know your thoughts and what experience anyone has in coding services of this nature. Thank you!
