This might be a stupid question. In order to bill for a procedure do you need an order for that procedure? For example, a patient comes in for irregular vaginal bleeding, the provider finds a cervical polyp and obtains permission to remove it during that visit. The polyp is sent to pathology. Does the provider need to add an order for the removal of that polyp?
