For ancillary office services, such as in-office testing and injections, like allergy shots and biologic injections, whose NPI goes in box 24J on the claim form? The ordering provider (provider on record) or rendering/supervising provider (provider on site)? Assume the ordering provider (provider on record) is not in the office and there is another provider present. Box 33 (billing provider) is the group as the billing is under the group's TIN.

Box 24J is only for the rendering provider (or supervising provider if it is not a face-to-face service with a physician or NPP).

Ordering provider information would go in box 17.
