Left knee genicular nerves radiofrequency ablation under fluoroscopic guidance to 3 target sites. I have coded 64640, 64640-51, 64640-51, 77002-26. It is also noted that one additional lesion was performed at the superior medial site. Is there a code that would cover the additional lesion ablation? If not, would this warrant a -22 modifier?
