Am I right in my thinking that if a dr has recommended orthotics for a patient, and the patient reappoints to be casted for the orthotics ONLY ... we can't charge for an OV at the casting appointment, can we?

You are correct, only bill for the casting procedure - an office charge would not be warranted.
