osbservation/inpatient billing

Insurance is denying claim because pt was in observation for 2 days and then admitted to inpatient what is the best way to bill for this???

You cannot have an observation and inpatient on the same day> what we do is IE pt in obs for 2 days ,MD decides to admit full we do not use the 2 day as pos obs we use inpt with the minimal admit code.
