Certain employess at one of our occupational health sites are required to fill out these questionnaires and they are sent to our site for review by our clinical staff. As per OSHA guidelines, respirator fitness can be determined by this mandated questionnaire. If the employee answers "yes" to any part of questions 1-9, they must come into us for a physical exam. What CPT code(s) (if any) can be used for the review of these questionnaires especially for ones where the employe doesn't have to come in for the physical? Thanks.
