I have a question regarding dx codes in this ov note. If MD only uses the Osteo as the dx ( M81.0) but there is mention of history of fractures do I need ask them to add that dx as well or leave it at is? (See below)
Probelm 1:
Osteoporosis, postmenopausal, severe (ICD-733.01) (ICD10-M81.0)
Assessed as: Improved
#Hx of severe osteoporosis, with multiple fragility fractures (she has fractured every vertebrae between L5 and T12, as well as her right wrist).
#Completed Evenity; she is started Prolia today
Plan:
-Start Prolia
-Repeat DXA 1/2027
-Offered referral to PT to assist with balance; she declined
