Out of Net Billing to Patient

What are the Guidelines for billing a patient after the out of net insurance EOB pays?
If the EOB states to disallow the amount, is it okay to bill the patient that amount anyway since the provider is out of net?
I have recently moved and my coding books are still in storage. I want to be clear on these guidelines.

If you are out of network then it is okay to bill the patient. I have worked for several dr offices and some of them bill the patient and others just write off the disallow.
